"Informing next of kin"

About: Esperance Health Campus

(as a relative),

My mother fell and broke her femur in Esperance. She was taken by ambulance and admitted to Esperance Hospital. After x-rays she needed to be transferred to Perth for specialist orthopaedic management. My father was with her in Esperance and he eventually went home to wait news of her transfer time.

Early the next morning he’d tried to contact the hospital and there was no answer on several attempts. He went up to the hospital at about 6am to see if he could find out how his wife was going. The nurse in the ED said that she was unable to help him as she ‘couldn’t access the computer’. No attempt was offered or made to make a phone all to the ward on his behalf. My father returned home and continued to ring until someone eventually answered. He was then informed that my mother was flown out during the night. He was very upset to find out she had be flown out without even an attempted phone call to inform him.

Dear Informing NOK,

I am saddened to read about your father’s experience at Esperance Hospital and wish to convey my sincere apology. Communication is a key part of our service and to hear that there was such a big break down in this area for your father is very concerning to me. I would like to investigate what has occurred so that urgent remedial action can be taken.

I ask that you contact Margaret Smillie who is the Acting Operations Manager at Esperance Health Campus on 9079 8071 or email on margaret.smillie@health.wa.gov.au. This contact will assist us in ensuring a thorough investigation is undertaken.

Once again please accept my apologies for your father’s experience and thank you for sharing your story with us.

Kind Regards

Peter Tredinnick

A/Regional Director
