My mum (87) had a fall at home. She was taken to the Geraldton Regional Hospital where she spent time recovering from a broken hip. Thank goodness she was referred to the Transition Care Team.
The Transition Care Team have been amazing.
Every member of the team have been genuinely warm and caring, very professional, more than helpful, patient, encouraging and kind. They have provided a range of invaluable services, ensuring mum received quality personal care at minimum cost, in her own environment, helping to aliviate stress and further discomfort. The team has reassured us as family in times of concern and provided professional assistance where required. Mum has been supported and cared for by the team so well, I feel without the team her recovery would not have gone as smoothly.
I am so very grateful for the loan of specialised equipment (organised by the Transition Care Team) during Mum's recovery, again alleviating financial stress.
My heartfelt gratitude goes to each and every member of the Transition Care Team for the work they do and the wonderful care of my mum, my children's grandmother and their children's great grandmother.
