"Continuity of care"

About: Royal Perth Hospital / Physiotherapy Service Specialists in the Perth Electoral District

I'm incredibly grateful to be able to access physiotherapy services through Royal Perth Hospital for the management of a complex and chronic condition. However one thing that is continually a challenge is continuity of care. I've seen a number of physiotherapists, all excellent, but being passed to a different person for each round of physio makes communication difficult. Requiring me to explain my medical history each time and has resulted in health issues not being picked up until they're causing other issues. Communication between departments and specialists has likewise presented issues, particularly as staff in different departments are often only employed for a few days a week. This is no fault of the hospital staff. But rather seems an issue of organisation that could perhaps be improved by digital communication and record sharing between departments to allow for better long-term management.

Dear Physio patient,

My sincere apologies for the late reply to your posting. Thank you very much for your feedback on the quality of our Physiotherapy Staff, and for your suggestions for better digital communication and case record sharing between hospital departments. We are working with the East Metropolitan Health Service towards upgrading our electronic record requirements and your comments are good reinforcement for the need to do this.

I apologise for any inconvenience caused to you for the times you had to repeat your case history, and any delays in receiving treatment. In our public teaching hospital structure where staff regularly move or only work part-time, this happens from time to time and we regret any service continuity difficulties for you. I really appreciate your feedback and we will work towards electronic information sharing.
