"PATS"

About: Patient Assisted Travel Scheme - WACHS Goldfields

(as a parent/guardian),

I took my child's Yellow PATS form in like I have done so many times for over a decade. All was fine until I was ask if we are traveling by train or road. I replied by car. I was then told, sorry no fuel cards as we have ran out. How does PATS run out of fuel cards? Lucky we have a credit card & yes I will have to use it for fuel. But what would happen if we didn't? This is not the first time this has happened to us & so many other families here in Kalgoorlie.

Dear Mrs Frangi

Thank you for taking the time to tell us about your experience. I am sorry to hear that the Kalgoorlie PATS Office had run out of fuel cards and weren’t able to provide you with the cards you required.

We have now received further supplies of fuel cards, and ask that you please contact Susan Jennings-Hingston, A/Operations Manager, Kalgoorlie Hospital on 9080 5812 in order to arrange a fuel card to be provided to you or a reimbursement if required.

Following your feedback, we are undertaking a review of our process to enable to prevent this from happening again in the future.

Kind regards

Carol Erlank
