"Ongoing Care After Hospital Discharge"

About: Community Nursing (Bass Coast Health) Wonthaggi Hospital / Emergency Department

(as the patient),

I suffer from Chronic Obstructive Pulmonary Disease (COPD) and often wake in the night unable to breathe. About a month ago I rang for the ambulance. It arrived at my door by the time I reached the door to unlock it!

I was given immediate attention in the Emergency Department and then moved to a hospital ward. After two days and nights of care I was discharged. This discharge happened suddenly as there was fear of a flu outbreak.

When I arrived home I received a call from a district nurse. This was so helpful and I valued their care over the past month. Each nurse was positive and extremely caring. Mistakes in medication were sorted and now I am ready to be on my own again.

Dear oscarqf93,

It sounds like you have had a pretty horrid time over the past few weeks - I am so pleased that you have been able to get home and become independent again.

Our Community Nursing team provides a wonderful service and I agree that each of them is very caring. I will make sure I pass on your feedback and thank you for taking the time to tell us your experience. I hope things go well for you.

Kind Regards

Jan Child

Chief Executive Officer
