About: Sir Charles Gairdner Hospital

(as a relative),

My spouse and I recently travelled 370kms from Exmouth to hospital see the visiting Ophthalmologist. My spouse had been seen at Sir Charles Gairdner Hospital (SCGH) in Perth a few weeks prior after a referral.

On seeing the Dr at the hospital, my spouse didn't have any correspondence from SCGH to follow up with. SCGH had not forwarded any information to the hospital on their consult which they said they would do.

We were very disappointed as we had travelled a long way with no real outcome from the visit. The dr was unable to write the prescription for my spouse's injections as they didn't have the details at hand. Then we had to drive the 370kms back to Exmouth.

Dear gigglenhoot,

Thank you for taking the time to share your concerns regarding your spouse’s recent experience with Sir Charles Gairdner Hospital (SCGH). I was very sorry to read that the information required for the outpatient appointment was not available and that because of this the required treatment could not be prescribed. I can only apologise for the frustration you both must have felt having travelled from Exmouth to access care only to discover that correspondence from SCGH was not available.

I will forward your concerns about the lack of communication from the outpatient appointment to the Ophthalmology department as it is important that we learn from your feedback. We are continuously working towards improving our processes as we want our patients to have a positive experience when accessing services.

If you would like us to investigate further I would encourage you or your spouse to contact the Patient Liaison Service at SCGH on 6457 2867 to enable us to assist you further.

Once again, my sincere apologies for your experience of our services

Kind regards

Janet Zagari
