"Lack of communication"

About: Fremantle Hospital and Health Service

(as a service user),

Received a letter from Fremantle Hospital for an outpatient appointment. Due to COVID-19, I rang the outpatient number on at the end of last month (1300855275) to check if my appointment was still on, as I believe elective surgery was being cancelled.

The person I spoke to said the appointment was still on. I turned up at the Fremantle Hospital for my appointment, only to be told that it was cancelled and someone should have rung me, which they hadn't.

Dear CaptainA

Thank you for your feedback regarding the cancellation of your appointment at Fremantle Hospital. Unfortunately, without further information, we cannot identify the reasons why your appointment was cancelled and why the miscommunication occurred.

If you would like us to investigate your concern further please contact the Fiona Stanley Hospital Patient Family Liaison Service on 6152 4013 or via email: FSHFeedback@health.wa.gov.au. Our usual process is to inform patients of cancelled outpatients appointments either by letter or telephone depending on the time frame, I am sorry this did not occur in your case and sincerely apologise for the inconvenience this caused you.

Kind regards,

Neil Doverty
