"Rehabilitation in the home team"

An elderly friend had a fall and received treatment at the ED at Armadale Hospital.

While in the department he was referred to the RITH team. Jeanette (OT) visited him at home on the weekend, and the following week there were visits and contact from physio, Mel (MSW) and the dietitian.

All the practitioners were very helpful and took the time to find out the whole picture of what was happening and assisted his carers to access further resources as well as facilitate communication between external providers.

Their support, professionalism and caring response was very much appreciated. I feel we are very lucky to have such a dedicated team in the area.
