"Discharge information"

About: Armadale Hospital / Colyer Surgical & Medical Ward

(as the patient),

I was discharged from Armadale (WA) hospital on a weekend morning with, in my opinion, inadequate medication and instructions following a lumpectomy. The prescription was for 3 tablets Celebrex (200mg instead of 100mg), 2 somac (40mg) and 5 Tramadol (one tablet every 4 hours), which was incorrect and insufficient. Each was a full prescription cost (n=10) from my local pharmacy. 

I believe that for people on a low income this is prohibitive. I also received differing discharge information about self care and apart from the discharge notes, nothing was written down for me to refer to later.

Thank you for taking the time to share the details of your recent experience with us.

Firstly, I would like to apologise for the lack of appropriate and effective discharge information provided to you. The experience which you have described does not align with our normal practice or standard for patient discharge and I would like to reassure you that the Nursing Coordinator (Surgical) is conducting a review to understand why this occurred.

Furthermore, it will be addressed with all staff the importance of effective communication with patients, particularly around discharge planning.

I was extremely concerned to read of your valid feedback relating the price of medication prescribed following a hospital admission. We are acutely aware of the importance of ensuring all our patients have equitable access to any treatment required to ensure they have an effective recovery.

At AHS we strive to involve patients in all aspects of decision making, including discussions to assist patients to make informed decisions about medications prescribed to them. These discussions should extend to ensuring the patient has the option of having their medications dispensed from either an external pharmacy or from our hospital pharmacy where the costs are capped for affordability. I would like to express my sincere remorse that this was not your experience.

I am extremely concerned by the experience you have described and would appreciate the opportunity to investigate your case specifically.
