"Vaccination certificate error"

Unfortunately, RPA vaccination hub is a failure based on my experience. I've been trying for the past 3 months to fix an error in my vaccination certificate. It seems my 1st jab has been placed on my spouse's certificate. They have been vaccinated 3 times! I am fully vaccinated but my certificate shows one only. I'm a front line health care worker and it's now impossible to do anything as we are out of lockdown. I've spent hrs on the phone and no one's helping.
