"My mother's transitional care post a fall and emergency surgery"

About: Prince of Wales Hospital / Transitional Aged Care Program

(as a relative),

The support and care my mother received from the Prince of Wales  transitional care team was just fantastic.  Each and every one of the team were courteous, respectful and, most of all, very caring. Their help was instrumental in my mother's recovery.  Even now, months later, my mother is still receiving support. Just a wonderful service.

My family can't thank them enough.

We will ensure that your kind words of thanks and gratitude are shared with the TACP team. It is reassuring to hear that the Team were courteous, respectful and caring in helping support your Mothers recovery post surgery, I am sure that your feedback will make the team very proud. The TACP team are highly skilled professionals that pride themselves in providing patient focused individualised home rehabilitation. Your feedback has helped us understand how important the TACP service is to our patients and our community.
