About: Sydney / Sydney Eye Hospital

(as the patient),

I was at the medical records department at Sydney & Sydney Eye Hospital and was attended to by a manager there called Lynda. I was applying to receive my medical records and Lynda went above and beyond in helping me with the form. 

One example is making me aware that since I am paying for 80 pages, I might as well apply for all medical records that I have with the hospital, when initially I had only stated on the form that I wanted my records only for 2017.
