"My Mothers End of Life Patient Journey"

About: Broome Health Campus / General ward Broome Health Campus / Palliative Care

My mum was Palliative Care and was being cared for at home.   The Palliative Care team, Aged Care and her GP were absolutely outstanding in the comprehensive and compassionate care that they provided to ensure mum and her carers were supported with every stage.   There was a high level of communication and extensive home visits by all health professionals.   The home visits included medication reviews, symptom management, equipment needs, emotional support and my family felt very supported and felt that we could contact them at any time.   My mum eventually needed to be transported to hospital for end of life care and the Palliative Care team coordinated the transfer including a comprehensive clinical handover to the hospital to ensure that Mum's patient journey was streamlined.   They also met mum at the hospital which was really lovely.   The hospital staff were all very caring and compassionate.   The Palliative Care room was of a high standard.   Only suggestion is to remove the "clouded" windows as we would have loved to be able to see the blue sky without having to leave the veranda doors open.  It made a big difference to mum and the family to have this link to the outside world.   A very big thank you to all staff involved and also coming to see  our family after mum passed away.

May I first extend my sincere sympathy to you for the loss of your dear mum.

I want to also say thank you for your generosity at this sad time to take the time to share your story and provide such positive feedback about the WACHS Palliative Care team, the Aged Care team and your mum's GP. I know they will be pleased to know their care and the care of the hospital team at the end of your mum's life was in keeping with her wishes and those of you and your family.

Thanks too for your comments about the standard of her room in the hospital and your suggestion about being able to see blue sky from the window. Perspectives such as yours will help us to continuously strive to improve the physical space within our hospitals for our patients and their families.

Bereavement after the loss of a loved one is a journey in itself and I hope you and your family can feel reassured that a member of our Palliative Care team will still be available to talk to you any time should you need some support.

I see that Carmen has responded to your post and I would like to echo her sentiments. I am really very sorry for your loss and truly appreciative of the fact that you thought to post your feedback and suggestions during such a difficult time.

I just wanted to let you know that after seeing your comments regarding the frosting on the windows of the room your mother stayed in, I spoke with our clinical and maintenance teams to see what we could do to make a change.

Following this discussion, we are progressing works to remove the frosting from the top half of the windows so that patients can see the sky whilst also retaining their privacy.

I trust that this change will improve the experience for future patients using the room. We would not have done this without your feedback. Thank you again.
