"Mother's visit to outpatients"

About: Esperance Health Campus / Emergency Department

(as a relative),

My mother had a trip in the ambulance one night recently to Esperance Hospital due to a fall at home - which resulted in a laceration to her head that required stitches. Mum had her head stitched and was observed and given a thorough check over for a couple of hours before being released that night. The staff were wonderful and her care was awesome.

There is only one concern which we didn't notice on the night. When she cut her name tag off the next day it was the name tag of another patient that was in outpatients that night (it was the name tag of the patient next to her -  as I knew the patient that was next door to her). Even though there was no complications as a result of this I just thought it would be good to mention it as you cannot improve things if you are unaware of any issues.

Dear rainrz89

Thank you for the encouraging feedback regarding the care your Mother received while at the Esperance Health Campus and we hope she is making a positive recovery.

It is very concerning for us that your Mum had the incorrect identification band and we appreciate that this has been bought to our attention. The service regularly carries out auditing across the hospital to ensure patients are correctly identified. Correct identification is vital for safe patient care. Immediate communication has taken place with staff to reinforce the seriousness of such an error. To assist us to validate the medical care of all patients during your Mothers visit, it would be helpful if you could contact the Director of Nursing, Carla Jones on 9079 8161.

I really appreciate you bringing this to our attention.

Kind Regards,

Peter Tredinnick
