About: Prince of Wales Hospital / ASB 3S - Community Management Centre

I just want to express my deep appreciation and gratitude for the remote monitoring services provided by the Community Management Centre at Prince of Wales Hospital. I had no idea such an amazing service even existed until my wonderful GP referred me to be monitored for COVID. 

For several days, this registered nurse Susan called me regularly. Her dedication and patience in monitoring my condition and guiding me through my COVID symptoms were truly remarkable. She consistently approached my COVID concerns with her wealth of knowledge and genuine care.

I cannot emphasize enough how comforting it was to know that help was just a phone call away when needed. This incredible service allows people like me to stay safely at home, reducing the need for hospital visits.

Thank you once again, Susan, for the kindness, understanding, and support you provided. It meant more than words can express.

Keep up the good work to the entire Community Management Centre team.

Thank you for your wonderful feedback for the SESLHD Community Management Centre (CMC).

It is great to see the safety you felt being supported in your own home virtually by the CMC team, allowing you to remain in your own home and not need to come to the hospital at all.

What is heart warming, is the person centred care the CMC team provided during your time being cared by the team. Your kind words will be passed on to Susan.
