Our elderly mother experienced a fall that led to admission at RPH, requiring five hours of facial surgery and two weeks of acute recovery in hospital. She was subsequently discharged to our home and referred to Rehabilitation in the Home (RITH) with the goal of regaining enough independence to return to her Community Living Facility.
During her three weeks at home, she was seen by three health professionals:
•Chris, the physiotherapist, conducted an initial assessment and then visited weekly, and provided structured exercises aimed at rebuilding strength and improving mobility. He supplied written guides for these exercises and offered instructions to both mum and us. Chris’ approach was very supportive and positive.
•Krista, the speech pathologist, held a phone consultation and provided information regarding strategies to assist with swallowing, which mum found difficult post-injury and surgery. The advice was helpful.
•Caitlin, the dietician, visited once to advise on meals and nutritional supplements. The advice was helpful.
Having mum discharged home with us was a little confronting and challenging. However, we found that having initial contact from RITH within 48 hours, with in-home visits within the next couple of days, to be supportive and helpful. All of the advice was beneficial to mum’s recovery and helped support us (as we supported her).
After three weeks, mum returned to her Community Living Facility. The guidance from the RITH health team was professional, supportive and helped mum recover quickly while supporting us. Thank you.
