"Discharge from Sir Charles Gardner Hospital"

About: Sir Charles Gairdner Hospital

(as the patient),

I’ve been in Charles Gardner Hospital for 2 days receiving treatment and care as I have Addisons disease and went into crisis due to me catching Covid. They were absolutely great the entire time. All the nurses, doctors, help staff etc.

But discharge and trying to get a medical cert. for my absence from work was horrendous and I felt they basically just forget about me and had me standing in the med ward for an hour and no one made eye contact any further once they were done with me. I finally flagged someone down and they said we'll look into it.

The nurse walked back a few minutes later and said they can’t get ahold of the doctor so I can just call my GP. I’ve stayed here twice for medical issues and never once had an issue getting a medical cert which I need to know when I’m fit to return to work and my work requires one. Overall happy and think this is a great medical hospital but pretty dissatisfied with how they basically kick you out once you’re healthy without all required forms and information.

Dear travellergt68,

Thank you for taking the time to provide your feedback about your experience at Sir Charles Gairdner Hospital and for acknowledging the staff for the great care that you received.

I was genuinely sorry to learn about the difficulties you faced in obtaining your medical certificate upon discharge. I understand how vital this documentation is, not only for your personal records but also for accessing follow-up care and workplace requirements. I’m disappointed that this process left you feeling forgotten and dissatisfied, and I want to extend my sincere apologies for the distress this caused.

We strive to ensure that every patient receives all the information and documentation that they require, especially during discharge, a moment that should feel reassuring and well-supported. It’s clear that we did not meet those expectations in your case, and I deeply regret that.

Please be assured that I take your feedback very seriously and have shared your story with the relevant team to help improve our processes and ensure that similar issues are avoided in the future.

If you have not yet been able to obtain a medical certificate from your GP, please contact the Clinical Nurse Specialist from the ward which you were discharged from as they will be able to provide the necessary assistance.

Kind regards,

Jodi Graham
