"Incorrect records"

About: Armadale Hospital / Emergency Department

(as the patient),

Incorrect records being passed on or omitted in reports being sent to my primary GP and to cardiac specialists that I depend on for my ongoing cardiac health e.g. one admission to ED for life saving treatment has not been documented on any discharge forms from later dates, it has been over one month since and my GP my Cardiologist and my Haematologist have not received any information of that event. 

I am very angry and frustrated because this has happened several times this year
