"Support from SESLHD Intellectual Disability Palliative Care CNC"

I am writing on behalf of myself and all the Vision Team.

I want to provide feedback on how much the guidance and support from the SESLHD Intellectual Disability Palliative Care CNC has meant to us for the last 18 months. She provided steady care, guidance and space for dignity. She gave me and my team confidence at a hard time in our resident's care. She made a real difference in the care we could provide and ensured we could keep our resident comfortable in her home. She is truly one of a kind and we could not be more grateful.
