About: Fiona Stanley Hospital / State Rehabilitation Service Wards A&B, Wards 1A& 2A

(as the patient),

I previously wrote as I was frightened of being discharged without a plan and was feeling frightened and teary. 

I want to thank you so much for your prompt response, as well as the Unit NUMs and the Medical Team treating me! 

The Medical Team and the Physiotherapists and O.T are working with me and have come up with a future plan around discharge. I feel I have time to safely work further on my goals and to face  complications with my injury and going home in the future to my living environment.  Thankyou to the wonderful Dr, physios, O.T's, NUMS and Nurses. And Thankyou again to the prompt response to my previous post. I feel relieved and grateful.

Thank you for taking the time to write to us with an update on your experience regarding your discharge from the Fiona Stanley Hospital (FSH) Rehabilitation Unit. It’s wonderful to hear that you feel reassured and supported, and that the Medical Team, Physiotherapists, and Occupational Therapists have worked with you to develop a clear discharge plan that meets your needs.

Your kind words about all of the staff who played an important role in your recovery is greatly appreciated. I will ensure your feedback is shared with the team involved and I know they will be delighted to hear the positive impact their care had on you.

Should you have any further queries or require additional assistance whilst you remain an inpatient, please do not hesitate to contact the Nurse Unit Manager, who will be happy to provide you with continued support.
