The input of the SESLHD Intellectual Disability Palliative Care CNC was integral in helping Peter’s NDIS supports navigate the hospice/end of life process. An element totally unfamiliar to the team. Without Maria Heaton's involvement we wouldn’t have had the direct line of communication to Calvary and miscommunications between stakeholders would have surely occurred.
Maria's patience, understanding, and accommodating of Peter’s complex support needs in situations completely unknown to him vastly contributed to his ability to cope. Being so available to Peter and having visited him at home before hospital would have made a major impact in being so comfortable in hospital.
It was a great experience working with the SESLHD Intellectual Disability Palliative Care CNC.
