"Failure to Carry Out Specialist’s Request for Test"

My Specialist referred me for multiple tests (urine, blood and another urine test requiring 24 hour collection) most of which were carried out a few weeks ago however, the location I attended did not have the specific container for the final urine test to provide to me, so told me they would order it in that day & advise me on its arrival.

A week later I called to ask re: availability of container but was advised their stores order did not get delivered but they would ask a nearby PathWest location if they had a container and either way would call me back. I received no call back.

I phoned again recently and was told they had re-ordered container last week.

A few minutes later I received a call to say container had just arrived and I can collect it now.

As I was contemplating needing to defer my follow-up appointment with the specialist due to him not having full results back in time for 2 February, I was very upset as it may be weeks before I get another appointment with specialist.

I am not sure who was to blame for non-supply of necessary container to location I will be re-attending, but am raising this matter to prevent other patients having to undergo this stress.

Not wanting to become a nuisance to the local staff but still needing to receive some or any communication on status of my enquiry, was difficult.

It appears after 2 weeks elapsing since my other tests having been completed, this may have now been resolved now, but I feel still worth reporting.

Please accept my sincere apologies for the delay and for the communication issues you encountered while waiting for the 24‑hour urine collection container. I understand how important it is to complete your testing, especially with your upcoming specialist appointment.

After following up with the Kalamunda team, I was advised that the specialised 24‑hour urine collection container required for your test needs specific laboratory preparation and has a limited shelf life. As such, these containers are delivered to sites only upon request and may not always be immediately available. I apologise that this was not clearly communicated to you, and that the delay in this instance was longer than usual due to miscommunication regarding the container type.

Staff have since been reminded of the need for clear and timely communication with patients.
