"Concerns regarding assessment and discharge"

I am writing about the care my child (who is a young adult) received after presenting to hospital following a serious assault.

On the night of the incident, they attended the hospital after being attacked by multiple individuals, including being strangled. They were in significant pain and had visible facial injuries.

Despite explaining what had happened, both my child and their partner felt dismissed. The nurse’s manner was perceived as rude and lacking compassion, and the doctor did not appear to carry out a thorough assessment of his injuries.

My child was discharged, given only paracetamol and an antibiotic, and was made to walk home, with instructions to return the following morning. This was particularly concerning given they had reported strangulation and significant facial trauma.

The following day, when they returned, their condition was reassessed and taken much more seriously. My child transferred by ambulance to another hospital, where it was confirmed that they had sustained a broken jaw in two places, requiring surgery.

The contrast in care between the two presentations was significant. While we are very grateful for the professionalism and compassion shown by staff the following day, the initial response has caused considerable distress.

This experience raises serious concerns about the assessment and management of patients presenting after violent assault, particularly where strangulation is involved.

I am sharing this in the hope that it leads to reflection and improvement in how such cases are handled, so that others in similar situations receive appropriate and timely care from the outset.

Thank you for taking the time to share your concerns regarding your child’s recent experience. I would like to acknowledge the conversation we have already had and thank you for meeting with me to discuss this matter. I am truly sorry for the distress this situation has caused both you and your child.

At WA Country Health Service (WACHS), our values especially community and equity, underpin the care we strive to provide to every patient and their family. I am very sorry that your child’s initial presentation did not reflect these values, particularly at such a vulnerable and frightening time following a serious assault.

I acknowledge your concerns regarding the perceived lack of compassion, the feeling of not being heard, and the adequacy of the initial clinical assessment. Presentations involving assault and strangulation are complex and require careful and thorough assessment. It is understandable that the difference in care between the two presentations has caused you concern and distress.

While I am pleased to hear that your child received appropriate care and compassion during their subsequent presentation and transfer for definitive treatment, this does not lessen the impact of your initial experience. Please be assured that your feedback is being taken seriously and will be used to support reflection and ongoing improvement in our clinical assessment processes, communication, and patient-centred care.

Following our discussion, we have already commenced ongoing improvements and discussions with the clinical teams involved. This includes reinforcing expectations regarding thorough assessment of trauma presentations, as well as the importance of clear communication and compassionate, patient-centred care and also the requirement for ensuring our patients safety.

As discussed, we are committed to ensuring our staff uphold WACHS values in every interaction. Your feedback provides an important opportunity to reinforce expectations around respectful communication, thorough clinical assessment, and responsiveness to patient concerns, particularly in cases involving trauma and violence.
