(as a relative),

I just want to share some feedback on behalf of my mum.

First of all, on behalf of our family, I would like to sincerely thank her cardiologist, Professor Sze-Yuan, for giving Mum the opportunity to be monitored through this heart failure app, despite us living outside the district area.

Secondly, I would also like to thank Nurse Susan from the Community Management Centre at Prince of Wales Hospital, who visited our home and helped Mum get set up on the app.

She is precise, concise and thorough in everything she does.

We are taking things one day at a time, and Mum is just very fortunate to have such wonderful monitoring in place for her heart failure.

I am so pleased to hear that your mother felt supported and well looked after both through admission and after discharge. It is so lovely to hear that she was provided with personalised support regardless of living within the district or not.
