About: Daylesford Hospital / Perioperative services

(as the patient),

Colonoscopy in newish theatre at Daylesford Hospital. Excellent new and clean facility with very friendly and helpful staff. Only issue was that the paperwork prior to admission did not capture my GP's current practice and on the day staff assumed on multiple occasions an incorrect practice address, one the GP had left 5 months prior. Otherwise, this feedback form is not straight forward at all.

I was particularly interested in your comment that "this feedback form is not straightforward at all."
