"Compliments to the multidisciplinary team at Fremantle Hospital 8S"

About: Fremantle Hospital and Health Service

(as a relative),

My mum was admitted to hospital a few short weeks ago and the last two weeks in Fremantle hospital on 8S before she passed away yesterday. I want to thank the multidisciplinary team for their care and compassion as they looked after mum and supported the family particularly as it became clear that she would not be coming home.

I want to thank the nursing team, RN, EN, AIN and students as they took time with mum and just reinforce that small actions mean a lot to the family and friends. The little things that made a big difference were appreciated. Thank you.

The team of non health professionals were also consistently caring for mum and bringing light and warmth when bringing meals, cleaning the room, taking meal orders or cups of tea or coffee. The cheery disposition by all was lovely support. Thank you.

Thank you to Dina, the ward clerk for helping us to catch key team members or help us to navigate the system and always greeting us with a warm smile and welcome.

The many medical team members that worked with family to ensure mum was comfortable and supported her pain management, Thank you.

There were too many of you all to mention by name and some were only in our team for a day/shift or two. My experience of the health system over these weeks will stay with me and my family in years to come. It is a complex system to navigate and help coordinate care for loved ones when they are their most vulnerable and we will forever be grateful to those that helped ease the pain and suffering for mum.

Thank you
