I want to say how grateful I am to my Team Care Coordinator, Annette who arranged several services for me including domestic assistance and funding towards an occupational therapist. Annette seemed to listen with care and compassion where others, I feel, saw my condition and its impacts on daily living as just another number in the system rather than an issue with a human being. Where Annette cannot directly assist, she can direct you to other services who can. I feel she is also a tireless advocate for the patient. I can highly recommend the Team Care Coordination through Medicare Local at North Lakes.
