(as the patient),

I am a care coordination patient who is very happy with the service provided by AHSICHS and my care coordinator. My health and lifestyle has been greatly benefitted by having one on one care and the supply of a CPAP machine. I have more energy and am less likely to drop off to sleep when watching TV. The quality of my life has improved and I am aware this has given me an opportunity to have a longer life span. I am actively enjoying having the ability to continue with my hobbies.
