I was a patient who was not great at managing my chronic disease, to the extent this was impacting further on my general health. After connecting with Lisa, I was forced to care about my health, and I have since been linked in with relevant specialists and allied health. Without Lisa's support and care I would not be managing my health well.

Lisa keeps in contact with me to ensure all is going well and prempts what service, assistance I may need. This is an invaluable service and one that should be continued, to ensure our mob's health continues to improve.
