My husband, and myself were very impressed with the Professional Assessment carried out by your Team Care Coordinator. We had no knowledge that such great assistance was available.

When my GP advised me to have an assessment, my thoughts were 'surely I'm not that needy', then here goes all my mats. However, Helen's visit put everything into perspective. Her advice was invaluable and reassuring. It is difficult to accept this wonderful service, but Helen convinced both of us that life hasn't changed, it is just different.

We are truly grateful for the wonderful service. Thank you.

I am so pleased that you have found the Team Care Coordination program of value and reassuring.

Our health system can be very difficult to navigate and our Team Care Coordinators find it a privilege to help people become aware of what is available and to assist you to access services that help keep you safe and independent in your home as long as possible.
