"HARP/Life Assist help for my elderly brother suffering from dementia and living alone at home."

After period of time with seemingly no immediate solution to resolving my brother's issues (he was in complete denial that anything was wrong with him) and contact with numerous people/agencies, we were put in touch with Caroline - a HARP Care Coordinator and Life Assist Consultant.

Since our initial meeting with Caroline (mid last year), she has been fully involved in establishing a strategy of transition from home to aged care. We were advised from the outset that there was a high risk it would not work because of his 'non-compliance'. That has certainly proved to be the case and we are no closer to convincing him to move.

Caroline has indeed been most supportive throughout and has initiated a number of strategies, including the appointment of a Guardian, via the OPA and securing a level 2 Homecare package.

As of now her role as a short-term coordinator has come to an end and the responsibility of overseeing my brother's long-term care etc, now passes to someone else. The entire day-to-day process remains a real challenge to us all and we are most grateful for the guidance and care shown to both my brother and ourselves, by Caroline.

Many thanks for taking the time to provide your feedback on Patient Opinion. From reading your feedback, this has certainly been a very difficult time for you and your family.

It is very evident the efforts Caroline has made have eased the stresses of this period and set things up for the future care and services needs.
