"HARP"

I work as a Case Manager with homeless clients and those at risk of homelessness  and through this  work have had several patients involved with  DHHS (Department of Human Health Services) Complex Clients.

Within this  process I have had two clients referred to the HARP (Hospital Admissions Risk Program) which has assisted in one case in the provision of numerous much needed assessments such as Neuro Psych, Geriatrician  and Psychiatric.

HARP assessments have been conducted in a timely fashion and they have been very non-judgmental which has been very helpful to the patients. In turn this has also been helpful to me, because I have had one site to attend and the patients have been really happy with the services provided to them.
