"Poor communication"

The patient was recently transferred by Patient Transport to Monash Medical Centre Clayton for an X-ray of two joints for investigation following an unwitnessed fall from bed at their aged care residence 3 days earlier with increasing pain.

The patient has cognitive impairment and is bed-bound, as explained in the letter from the patient's GP, and a copy of their drug chart was also sent.

X-rays were taken and no obvious fractures were detected, diagnosis of limb pain given. Would be interested to know if this was from the tendon contracting, but no explanation was given.

I am very disappointed that after more than twelve hours in hospital the patient was transferred back to their aged care residence without any contact or handover to nursing staff, who were only aware of the patient being returned when the Patient Transport arrived at the residence.

Surely this is not best practice?
