I am very upset, after 6 weeks of care, including weekly blood transfusions, my friend was taken home without any paper work or follow up service.
He chose to stay at home even though he lives alone, he is terminally ill, dying with liver failure. He prefered to spend the last days in his own home. Expecting some support as was promised him while he was there. I was even told that transport, community nurse and palliative care would be there to assist him.
The hospital did not arrange for anything, leaving him to fend for himself.
I rang the NUM to complain and express my concern, she sounded shocked and said she would investigate and get back to me.
I received a call later that day with a somewhat changed attitude from her. She defended the decisions made by her staff and said the hospital did not arrange for any after care and the patient had to do that himself. What?
We are talking about a dying elderly old man, who cannot walk properly, who lives alone and often suffers with confusion. Please answer this complaint.
"Poor follow-up care of a palliative patient following discharge."
