(as a relative),

Following on from an unexpected hospital stay Dad was provided with a Transition to Home Care Package from WA Country Health Service in Geraldton. We could not be more appreciative with this team of wonderful, friendly, helpful people.   Dad continually praises how warm and caring they are and we as a family we are blessed to have had the services provided to help Dad get back into being able to care for himself while staying at home.   Thank you all from the bottom of our hearts for the care you have provided to Dad.

Thank you for taking the time to share your positive experience on Patient Opinion. I was truly delighted to read your story and to hear that you and your father had such a wonderful experience with the service provided by the Transition Care Team. The Transition Care Team work with families and carer to help people like your Dad who have had a stay in hospital transition back to life at home. The work they do is invaluable and I know they will appreciate the recognition of a job well done.
