(as the patient),

2 days ago I got taken off soft food diet, but every meal I get is soft no matter who I ask. I keep getting told it will change but it doesn’t. 

Please stop serving me this mush, I’ll order out from now on.

Dear Disgrun,

Please accept my apologies that on this occasion we failed to deliver the standard we strive for, it is hard to know what went wrong for you but communication between teams sometimes is not at the standard we would like. Also, knowing who to speak to on the ward when things are not going well is important for our patients. We are working on improving our patient information, to make it clearer for you to flag issues with your care as they arise when you are a patient on the ward.
