My mum was Palliative Care and was being cared for at home. The Palliative Care team, Aged Care and her GP were absolutely outstanding in the comprehensive and compassionate care that they provided to ensure mum and her carers were supported with every stage. There was a high level of communication and extensive home visits by all health professionals. The home visits included medication reviews, symptom management, equipment needs, emotional support and my family felt very supported and felt that we could contact them at any time. My mum eventually needed to be transported to hospital for end of life care and the Palliative Care team coordinated the transfer including a comprehensive clinical handover to the hospital to ensure that Mum's patient journey was streamlined. They also met mum at the hospital which was really lovely. The hospital staff were all very caring and compassionate. The Palliative Care room was of a high standard. Only suggestion is to remove the "clouded" windows as we would have loved to be able to see the blue sky without having to leave the veranda doors open. It made a big difference to mum and the family to have this link to the outside world. A very big thank you to all staff involved and also coming to see our family after mum passed away.
