My parent was recently in the Royal Brisbane and Women's Hospital with a neuroinflammatory condition. On admission, and at every administration of any drug, staff were informed that my parent has an extreme allergy to mushrooms which causes an aggressive gastric response.
The kitchen staff were also informed at least half a dozen times. The dietician also visited my parent twice after the form that came with their meal stated no allergies. At that time, the dietician assured us that my parent's allergy was logged in the system.
That evening for dinner, my parent was served mushroom soup.
I feel this is completely negligent. My parent wears glasses and can't read without them and is not checking the form before every meal. If I had not been there and checked it myself, my parent would likely have eaten the mushroom soup and become incredibly unwell as a result.
