"Care by HARP team"

About: Hospital Admissions Risk Program - Eastern Health

(as the patient),

Due to ongoing illness & short stays in William Angliss, when I was discharged last time unbeknown I was put under the care of the HARP team & before long I was contacted by a diabetes educator, dietician & physiotherapist all wanting to see me.

I subsequently was visited by all & all trying to help me manage my illness. I must say that I was totally disappointed in one of the other HARP members who decided that it was their responsibility to make a call on renewal of my car licence & notified Vic Roads re same. I really was upset with this but due to some other factors, I surrendered my licence anyway.

I can only speak good words re the HARP nurses, they have honestly been just the best & they continue to visit me. The physio nurse tried to get me exercising which is important & my dietician is currently trying to help lower my blood/sugar readings & my dietician is doing their very best in trying to get me to eat more healthily & cut down on portion sizes & that has been quite a challenge for me but am doing my best. But now & then I eat more than I should & have some things which I shouldn't.

As for me, I have definitely been helped by the HARP team so a big thank you.

Dear octoberfw96,

Thank you for taking the time to share comments on your experience with the team working in the Hospital Admission Risk Program (HARP) at Eastern Health. I was so pleased to hear that it was largely a positive one (noting your comment regarding your driver's licence).

It will give me great pleasure to share your feedback with them and I hope the areas they are supporting you in are helping keep you well and at home (the place we all want to be!). I don't think you're any different to anyone else where we 'stray' at times, but as they say - all in moderation!!

I hope you continue to reap benefits from how the staff support you and I wish you the very best of health.

Kind regards and take care,

David
