I was admitted to Transition Care Program (TCP) post fall in the community. I was supported within the community and assisted to attend medical appointments. I was assisted with rehabilitation by the Physiotherapist and Allied Health. I was supported by the TCP Case Manager to maximise my independence to remain at home and keeping in touch with my progress. Thank you to TCP and the team at BCH I was able to remain at home after TCP.
