"Transitional Aged Care Team - Prince of Wales Hospital"

About: Prince of Wales Hospital / POW Community Health Prince of Wales Hospital / Transitional Aged Care Program

(as a relative),

Thank you to the amazing Transitional Aged Care Program (TACP) team at the Prince of Wales Hospital Randwick NSW. After two catastrophic strokes in the space of eight hours, we thought we would lose our beloved elderly parent. Miraculously, they came back to us. Following a six-week hospital rehabilitation, they were sent home under the care of the Transitional Aged Care Team from POWH.

The service given by these highly-skilled, dedicated and compassionate people was amazing, and we credit our parent's remarkable recovery to these fantastic health professionals. Even before my parent left the hospital, they visited and planned a comprehensive, individually tailored program of continued rehabilitation for them.

My parent had an Occupational Therapist, a Physiotherapist, a Registered Nurse, a Speech Pathologist and a Community Care Assistant. These wonderful women went above and beyond to redevelop skills affected or lost as a result of my parent's stroke, including a complex program to retrain bladder function and discontinue the need for a urinary catheter, during which the services of a highly skilled continence nurse were also called upon. This aspect of my parent's recovery made an enormous difference to them, both physically and mentally.

We made good friends with these fabulous women and enjoyed their visits and the excellent programs they devised for my parent's recovery. They have left them with ongoing skills to maintain and increase their level of health which they feel would never have been possible had we not had the privilege of their comprehensive care and knowledge.

Thank you TACP team – Annie, Kim, Belinda, Selene, Ally and Romina - from the bottom of our hearts! Our parent could not have done it without you!

My parent's very grateful family.
