"Disinterested and inappropriate staff in Bariatrics"

About: Sir Charles Gairdner Hospital

(as a relative),

After taking time off work and travelling up to Perth for an appointment re bariatric surgery and told you're basically too fat for fat reduction surgery and this hospital provides the surgery but not, apparently, the support or guidance to make you eligible for it.

In my opinion, this is heartless after being told by the patient they are suicidal over lack of assistance in the matter. The patient has had very expensive testing of hair, blood and urine for extensive investigations into reasons for obesity. These GP-ordered/suggested tests were seemingly ignored/not considered as an indication of the patient's want/will/desire to sort their problem. If this same person was in a motor vehicle accident, would they be told, sorry you're too fat for us to treat you? No, they would not, so why the 'you're too fat for fat-reduction surgery' when the patient has clearly tried other methods to help correct their obesity?

This patient had surgery in another hospital without mention of their weight. This patient has not been referred to a dietician or psychologist, as I believe is the norm with private bariatric surgeons. Why does this not occur at SCGH? Surely the whole of the patient should be involved in such a serious procedure?

This person is a hard-working, taxpayer who eats normal amounts, has tried other methods to sort their problem and wishes to be pain-free (having been saved after a serious MVA in 2000 and now has a right side and both hips metal), and can't get past first base. The appointment will be mentioned should this patient attempt to self-harm.

Dear Madmumma,

Thank you for sharing your feedback with us.

I am very sorry to hear that the appointment with the bariatric surgery team did not result in the outcome that the patient was hoping for and that this is affecting the patient’s mental health. If the patient is not already accessing professional help for their mental health wellbeing, I strongly recommend that they contact our consumer liaison service on 64572867, so that we can facilitate this process.

I understand that it would have been very upsetting to be advised to lose some weight before progressing to bariatric surgery.

All surgery carries risks, and decisions are based on each patient’s clinical condition, researched clinical practices and the best interests of the patient. It is the bariatric team’s responsibility to fully inform their patients of the options, risks and reasons for clinical decisions related to bariatric surgery and I apologise sincerely if the staff did not provide all the information that the patient needed. I am also sorry that the communication was not more caring and empathetic.

With regard to the hospital not providing dietician and psychiatric support for weight loss, the public health funding model requires weight loss pre- bariatric surgery to be managed within the primary health care setting in the community. This model usually enables such services to be accessed via a general practitioner, closer to the patient’s home base.

We strive to provide excellent healthcare services at all times and I apologise that this was not the patient’s experience. I will ensure that the bariatric surgery team is advised of your feedback.

Kind regards

Dr Theresa Marshall
