"Hospital not keeping their word"

(as a relative),

I shared information with FSH regarding my relative and what was going on for them. I revealed some of the things I located in their home and my concerns after my relative had told me they were making their own medicine.

I asked the first doctor and second doctor I spoke to not to mention this information to my relative as I was the closest family they had and it was important they only kept this for their own information so they could understand better what was going on for my relative to be able to help them.

Instead, I believe they accused my relative of being a drug addict and said that I had sent them photos. Then on my relative's discharge form, I believe they not only named me for the things I did say but also for many things I did not say.

Now my relative does not want to have anything to do with me, but more so, I believe they are now labelled as an addict, and not getting the help they need.

I am sorry to read of the concerns you have regarding confidential patient information.

The allegations you make regarding being named for things you did not say are serious in nature and we would like to properly investigate your claims.

Our Patient and Family Liaison Service (PFLS) coordinates feedback, provides information, advocacy and support to patients and their families, and can provide you with advice regarding rights and responsibilities. At this stage I think it would be best for you to make contact with PFLS so that we can properly assist and advise you.
