"Lack of discharge care"

About: Sir Charles Gairdner Hospital / Gastroenterology

(as the patient),

I was recently admitted to Sir Charles Gairdner Hospital for Acute Colitis caused by 2 bacterial infections including Salmonella. During my few days stay, my electrolytes were extremely low and I was unable to eat or drink anything (fluids intravenously only) until the day I was discharged. I was given a bag of medication to take with me on discharge, however was given no guidance as to diet or electrolyte management during my recovery at home from this serious condition. I am immunocompromised so I am at greater risk of serious complications. While the care I received was of a high standard, I believe I should have been provided with proper management advice on my discharge, beyond pure medication, in order to assist me in my recovery at home. While I am an informed health consumer, with significant experience managing my multiple co-morbidities, I did not leave the hospital feeling confident about how to self manage this serious condition and maximise my recovery.

Thank you for advising us that you were not given guidance about diet and electrolyte management before you were discharged. It is expected that all of our patients are fully informed about managing their recovery at home before they are discharged and I apologise most sincerely that this was not your experience.

As someone with multiple comorbidities, I can appreciate that you are an informed health consumer and I would like to thank you for engaging with us, to provide your useful feedback.

I will ensure that the staff are reminded about the quality of discharge we expect.
