About: Prince of Wales Hospital / Transitional Aged Care Program

(as a relative),

My mother had a fall, and ended up in hospital for about six weeks. We were worried about the transmission back to home and when released, needed the help from the Transitional Aged Care Program. The TACP was the perfect solution, and based on our experience, I would highly recommend the program and its staff. I need to mention the four ladies that helped my mother, and they were –

• Stacey – Case manager and Occupational Therapist

• Belinda – Physiotherapist

• Dianne – Nurse

• Allyson – Community Aide

All were professional, and very understanding, as my mother spoke very little English.

Thank you for your positive feedback from the recent experience both yourself and your mother have had with the Transitional Aged Care Program (TACP). The service not only aims to allow people to stay at home safely after hospital care, but also to meet the individual needs of each person which has been made evident from your lovely feedback.

Thank you for acknowledging the staff members who were a part of your mother's transition and care at home. Your wonderful words have been acknowledged and a special thank you provided to these members.

I hope your mother is recovering well and again thank you for your positive feedback towards the TACP service and the staff members.
