I made an inquiry to Sydney Hospital COVID-19 testing clinic regarding the result from my COVID test undertaken at the hospital in which I was verbally advised that the pathology sample was ‘’lost’’. It appears that, as I had not received the result from the COVID test, it was only a result of my inquiry that the hospital became aware that the pathology sample had been lost. This was following their consultation with another hospital that were undertaking analyses, apparently.
On several occasions, I requested the hospital to advise where it was, within their policy process, that the system failed and ‘’lost’’ the pathology sample. The language regarding the lost pathology sample has now become ‘’unable to locate’’. Either way, I feel the hospital has excused the failure of their system and have not provided a rationally persuasive explanation as to what led to the pathology sample being lost.
It took the hospital 16 days to write to me and still have not responded to my inquiry.
