"My father's feedback"

About: Swan Hill District Hospital / Sub-Acute Services

(as a relative),

My father asked me to write this feedback.

I want to express my gratitude to all the caring staff I have encountered within the sub-acute department. This was all staff I saw; nursing, doctors, Dr Keith, cleaners, physios, and dietitians.

I went to rehab & was transferred to palliative care. In my opinion, the service was "A+++". All staff I encountered were courteous, kind, attentive and nothing too much trouble. I was informed of every care decision and my family informed. I rate the service as one of the best I have been to.

Thank you.

Dear authenticwt48,

Thank you so much for taking the time to write about your father's experience in our Subacute unit. It is a reasonably new service, only opened in 2019, and we are very proud of the service it offers with the interdisciplinary team under the direction of our geriatrician Dr Keith. We are fortunate to be able to offer transitional care, rehabilitation and geriatric evaluation to the community in this unit, as well as inpatient palliative care, and we are very proud of this service and the whole Subacute unit.

Your father's story resonates so much because we at SHDH always try to provide "Connected Care, Best Experience", for every patient on every occasion, and the fact that he found the service A+++ is really wonderful acknowledgment.

I will be sharing your story with the wider Subacute team, thanking them for their hard work and caring, and am replying because I know our Nurse Unit Manager is taking a break currently, and she will be so proud when she returns to work to see this wonderful acknowledgment of the teams work.

Kind Regards

Chloe
