I was in Atherton hospital for several issues: I wasn't feeling well, had a bad headache, felt dizzy and when a clinician did my observations, my systolic BP was 195. When the clinician saw that, I recall they commented that it's very high, became alarmed and said they were going to talk to a doctor.

Around 45 minutes later, l rang my spouse because the clinician never came back and l was feeling worse. My spouse is a retired nurse and said to ring the nurse bell. The clinician came back and, as I understand it, told me not to worry about it. I told them l was worried because from what l knew, 195bp was the stroke or heart attack zone. I recall asking what steps were they going to take to reduce my BP. The clinician said nothing.

Long story short, around 6 1/2 hours later my BP was still 195 and l was hallucinating. l asked the next clinician what they were going to do about lowering my BP and I recall they said they were leaving soon and they didn't know anything about it. I asked them if they should discuss it at handover and they said yes. I told them l had documented what l could about my situation and, in my opinion, if l had a stroke or heart attack because of them or their colleagues' incompetence l would make damn sure they lost their jobs. Then they decided to get me some medication (tablet and an injection) I'm pretty sure to stop clotting and/or heart attacks.

Here's the kicker, l went to my GP and told them what went on. My GP looked at my hospital record and seemingly none of what l had just told them had been recorded. Nothing about my 195bp, my hallucinating, nothing.

If they erased records my question to you, is that a crime? Or failing to document patient records, is that a crime? Finally, when they gave me the injection and tablet, can they erase the record of having given me that and if they gave that medication, why do the records seem to not indicate me having a condition requiring them to give me that medication?
