"Incorrect discharge papers"

About: Royal Perth Hospital / Older Adult Orthopaedics (Orthogeriatric) & Ward 8B

(as the patient),

A few weeks ago, a driver had a medical episode while driving, arrested and crash into myself head on travelling about 80km/hr.

On the next day, I had surgery to fix 2 broken bones in my arm and had metal plates inserted as well. I was discharged from hospital a few days later, in the afternoon. When the doctor came to see me I asked for a medical Cert as I work fifo and eventually need to be cleared to fly. I recalled they said no worries, they will sort that and my discharge papers. An hour and a half later a nurse came back with discharge papers and no med cert, when I asked for it the nurse said the doctor should have done it but now they have rushed to lunch, and got another doctor to sign a medical cert for me. 2 days later after getting home, I reread my discharge papers and it states my age and gender as well as the sentence "admitted post MVA, car vs tree 60km/hr”.

As you can see from my story, this story is completely fabricated. I will also state that my age as stated on the certificate was incorrect by 4 years.

As to why there was such little care taken while writing my discharge papers which I believe are also legal documents.

I called to have this fixed asap and they said it can take up to 30 days to fix this.

For insurance purposes, legal documents and court purposes, I believe all these documents should be correct.

Dear omegafb94,

Thank you for taking the time to provide your feedback regarding incorrect and inadequate discharge documentation. Firstly, I am sorry to hear of the unfortunate accident you were in and trust you are continuing to heal well.

Secondly, I would like to express my sincere regret that your discharge from RPH was not satisfactory, with the wrong information recorded on your discharge letter, request for medical certificate not met at the time and the additional inconvenience for you to obtain amended paperwork.

This is not the level of care we strive to provide at RPH and I can reassure you your experience will be shared with the ward staff so they can understand the impact this had on you. It is important our documentation is recorded factually and in a timely matter and I am sorry that neither occurred for you.

You have indicated a wait of 30 days to receive your amended paperwork, which is consistent with formal complaint process timeframes. As you have indicated Orthopaedics as the specialty, I have requested they expedite any current requests they may have to ensure you receive a prompt response.

If this matter is not resolved over the next week or you have further concerns, I invite you to contact the Consumer Engagement Unit (CEU) via phone: (08) 9224 1637 or email: RPBG.Feedback@health.wa.gov.au.

Again, please accept my sincere apologies for your experience and I wish you well for your ongoing recovery and future health.

Kind Regards,

Dr Lesley Bennett
