"Discharge protocol"

About: Fiona Stanley Hospital / General Surgery Department & Ward 7A & Ward 7B

(as the patient),

I would like to know how many patients has Fiona Stanley discharged into hotels, motels or even hostels, when they are a country patient who has no one with them, after said patient has had sedation for a day procedure. I believe I have been told this is what will happen to me, because PATS will give me up to $100 for accommodation.

As I understand it, WA Health Department protocol is: After sedation patient must be monitored for 6 hours and then may be discharged into the care of another person. I believe you must have someone with you for 24 hrs after sedation!

Hands up if you have been discharged against Health Department protocol. When they do this to me, as I recall FSH Social worker has told me they will, then, because I believe a social worker has no idea of my medical or mental issues which are many and diverse. Because I feel a social worker has no authority that I know of, to be telling patients this. Mainly because I am sick to death of Health Dept policy and protocol being seemingly thrown in my face as an excuse for anything and everything, I am sick to death of feeling that social workers, nurses and even doctors are being turned into Government puppets, in my opinion. I am sick to death of being made to feel like I am just a number, not a human being that has been under health care constantly for 20 yrs and has seen the change happening. I believe city hospitals are no longer a caring place to be. It seems to me the staff, the front liners if you wish, are made to get those numbers up, or down, whichsoever the Board of the Hospital wishes. I feel patient care is particularly non-existent at Fiona Stanley.

I am thinking of approaching legal aid about these issues. I have a lot of pain issues after my procedure and I believe the hospital should be taking responsibility for my care in that 24 hours, I feel I will not be foisted off with no access to pain meds when I need them. My decision is I will sleep in my car, as close to ED as possible so that medical help is at hand if I need it.

Of course now, as I feel I am a number, there is the chance that I wouldn't get seen in ED either quickly enough and with my issues, if you read somewhere of a patient dying while waiting for treatment, I feel it may be me!

I believe a recent politician has changed Perth Hospitals from wonderful caring centres of healing to businesses that have to make money. You can hear the American health system knocking, in my opinion.

Dear DEADEND,

I am sorry that you feel that you are being treated as a number. We strive to provide excellent patient care, and have many hardworking clinicians and non-clinical staff who routinely provide excellent care. I strongly encourage you to speak in person or via phone call to your treating team regarding your feelings and concerns.

The discharge of patients from the Day Surgery Unit (DSU) who are admitted and discharged on the same day must meet criteria to be discharged safely and appropriately. There is an endorsed “Discharge Criteria for Day Stay Patients” guideline which Clinician Leads follow. No time limit is set for patients’ discharge, rather the patient must meet the required criteria. For patients who do meet the criteria for discharge on the day of surgery from the DSU;

- A responsible adult must be available for transporting the patient to and from the hospital (preferably by private vehicle)- A responsible adult must be available to remain in house with the patient overnight and until the next morning (preferably the same person will transport and remain with the patient)
- The patient and / or escort has a telephone at home, or mobile phone.

Without your details we are unable to comment on the interaction you had with a Social Worker. Based on the information you have provided you would not meet the criteria for same day discharge.

I apologise for any miscommunication or misunderstandings you experienced. I encourage you to contact Patient and Family Liaison on 6152 4013 so that we can complete a thorough investigation with the aim to improve processes and your overall experience.

We look forward to hearing from you soon.

Kind regards,

Neil Doverty

Update posted by DEADEND (the patient)

Ók, I have just received a letter dated 9 March supposedly from yourself. I feel that your first response about protocols being followed which you yourself said I do not meet that criteria, and the letter which states that as a busy hospital, you may not be able to allow my admission, I feel totally confused.

I don't want to stay in hospital, I need to stay there. My admission as overnight short stay should be made when appt. date is set. My health issues are extremely complex I have extreme pain after every change. I have suffered many and varied issues involving some part of the procedure in the past and know I will every change.

Why can nobody accept the fact that after 10 yrs of this, I know my body and it's reactions better than anybody. Why not try to ease my stress and just book the bed when appt made? Don't book it as a day procedure, book it as short stay, then at least I don't have that issue to think of.

Regards

DEADEND
